"DULUTH, Minn. -- A Minnesota woman has lost her lawsuit over a letter that erroneously told her that her son had died in Iraq. Joan Najbar of Duluth, Minn., sent a letter to her son, Sam Eininger, in September 2006. It was returned several weeks later, stamped "DECEASED.""

I found this article and thought I would pass it along.
"A wrongful death/medical malpractice lawsuit has been initiated against a Rochester assisted?living facility in connection with the death of a 78?year?old man two years ago who was allegedly assaulted by a fellow resident. Both had dementia."
